Coverage

Dmand AI covers 997K+ US sites of care in 21 site types, from hospitals and practices to pharmacies

A site of care is one place where patients are seen: a hospital, an outpatient department, a practice office, a clinic, a nursing community, an agency office or a pharmacy. Sites are counted as places, not companies, so a group with five offices adds five sites here and one group on its own page. Switch the list between care settings, or read the map for where sites cluster.

Site types

Dmand AI sorts its sites of care data into eleven groups of healthcare facilities, from hospitals and medical groups to pharmacies

Each group shows its count and what it covers. Six groups have their own data page, and every site type is searchable in the app.

Org hierarchy

Dmand AI places 641K+ sites of care under the health system, operator or medical group that runs them

An address tells you where care happens, not who buys. Each site in Dmand AI’s org graph points up to its parent, so a search for urgent care centers can return the health systems that own them, and one health system opens to its hospitals, clinics, nursing facilities and practices. Solo practices and pharmacies sit outside the graph and are searched on their own.

Questions

Sites of care and healthcare facility data FAQs

What buyers ask before they choose a source for data on US care settings.

How many healthcare facilities are there in the US?

Dmand AI tracks 997K+ US sites of care. They include 6.2K+ hospitals with a CMS certification number, 22K+ hospital outpatient departments and units, 429K+ medical group practice sites, 235K+ solo practices, 90.7K+ pharmacies, 61.2K+ physical therapy and rehab clinics, 59.6K+ nursing and senior living communities, 53.3K+ community clinics and urgent care centers and 23.5K+ home health and hospice offices. Counts as of October 25, 2026; other sources count facilities differently.

What is a site of care?

A site of care is the physical place where a patient receives care: a hospital, an outpatient department, a physician or dental office, a clinic, a surgery or imaging center, a nursing facility, a home health or hospice office, or a pharmacy. Dmand AI counts each place once, separate from the organization that owns it.

What are the main types of healthcare facilities?

Dmand AI groups US sites of care into 21 types: hospitals and their outpatient departments, medical group practice sites, solo practices, FQHCs, rural health clinics, urgent care, school and student health, government and tribal clinics, surgery centers, imaging and mammography centers, clinical labs, dialysis centers, behavioral health centers, physical therapy and rehab clinics, skilled nursing facilities, senior living communities, IDD residences, home health agencies, hospices and pharmacies.

What is the difference between a site of care and a healthcare organization?

A site of care is a place; a healthcare organization (HCO) is the company or system that runs one or more places. Dmand AI tracks 124K+ parent organizations, such as health systems, senior care operators, home care companies and health centers, and 181K+ medical groups, and links sites of care to them.

Can I find the parent organization of a clinic or facility?

Yes, for sites in the org graph. 641K+ sites of care sit under a parent organization or medical group in Dmand AI, so a clinic, agency or nursing facility opens to the system or operator above it, and the parent opens to all of its sites.

Which sites of care have no parent in Dmand AI?

Solo practices and pharmacies are kept outside the org graph, so they carry no hierarchy. Some clinics, agencies and senior living communities are not yet placed under a parent; their records still hold type, address, identifiers and clinicians.

Can I build a list of outpatient facilities in one state?

Yes. Pick the outpatient site types you sell to, such as urgent care, surgery centers, imaging centers and clinics, add a state, county or city, and save or export the list with each site’s parent, NPI and EHR.

Can I filter sites of care by EHR?

Yes. The EHR is a filter in the app, so you can list the sites of any type that run the system you sell into or replace, then open the parent of each one. The EHR and technology data page covers the filters.

Does Dmand AI include post-acute care providers?

Yes. Post-acute coverage includes 14.7K+ skilled nursing facilities, 39.7K+ senior living communities, 5.2K+ IDD residences, 14.4K+ home health agency offices and 9K+ hospice offices, each with its type and, where Dmand AI has one, its operator.

Can I see the clinicians and leaders at a site of care?

Yes. Sites link to the HCPs who practice there, from Dmand AI’s 7.4M+ HCPs, and to the leaders of the organization that runs them, from 11M+ healthcare executives.

Where does Dmand AI’s sites of care data come from, and how current is it?

Identity, NPIs and addresses come from the CMS provider registry, certification numbers and facility types from CMS facility records, and Medicare enrollment from CMS enrollment records. Site types, parents, clinician links and EHRs come from Dmand AI site of care data, org hierarchy data, affiliation data and EHR install data. Records refresh every month.

Is there a free list of US healthcare facilities?

Yes, for two site types. The Dmand AI hospital directory and medical group directory list sites by state and city with free profiles. Every other site type can be searched during the 30-day free trial.
