Build lists of solo offices by type and place, then open the clinician on each.
Independent practice data for 235K+ solo practices, with the practice and its clinician on one record
Find the one-clinician offices that group lists leave out: family doctors, dentists, therapists, chiropractors, optometrists and nurse practitioners who run their own practice. Each record shows the practice NPI, the practice phone and the clinician who works there, so your first call reaches the person who decides.

Coverage
Dmand AI covers 235K+ US solo practices in 16 practice types, counted apart from medical groups
A solo practice is one clinician seeing patients at one location under their own practice name. When two or more clinicians bill together, the practice is a medical group and is counted on that page instead. Switch the list between practice type, who leads the practice and Medicare enrollment, or read the map by state.

Solo practices, October 25, 2026. Each practice is counted once, by practice type, leadership and Medicare status.
Practice profiles
Every independent practice profile shows the practice, its one clinician, its Medicare status and the EHR it runs
One record per practice, keyed to the practice’s own NPI, so the office and the person who runs it never end up on two different lists.
The overview shows practice type, NPI, phone and address
Practice type and care area, the practice NPI, address, phone, website and whether a physician leads it.
The clinician tab shows who sees patients there
Name, credentials, specialty, individual NPI and training, plus any medical group where the clinician also works.
Medicare shows enrollment and payments
Medicare enrollment, the PAC ID and Part B payments for practices that bill Medicare.
Technology shows the EHR the practice runs
The EHR vendor and product on file, which also works as a search filter.

Solo practice view
Dmand AI keeps each solo practice as its own account and flags the clinicians who also work in a medical group
A solo office is easy to lose: lists built from groups skip it, and lists built from people bury it under the clinician’s other jobs. Dmand AI keeps the office as its own account, and 230K+ practices show the practice NPI beside their clinician’s NPI. When that clinician also sees patients in a medical group, as 81.7K+ solo practitioners do, the record links to the group, so you can sell to the office, the group or both.
Search
Build a list of independent practices by practice type, location, clinician, Medicare status and EHR
Filter practices the way your reps qualify a small office, then open the clinician on each record. Or start from people: search clinicians by specialty and keep only those who run their own practice.
Practice type
Care area, practice type and specialty, from dental and chiropractic offices to NP-led clinics.
Location
State, county, city and ZIP code.
Clinician
Credentials and specialty, physician-led or not, and whether the clinician also works in a group.
Medicare and technology
Medicare enrollment and the EHR the practice runs.
Contacts
Reach the clinician who owns the practice by practice phone, work email or mobile
In a solo practice the clinician usually makes the buying decision. Each record carries the practice phone listed with its NPI, and Dmand AI finds a work email or mobile number for the clinician when you ask, charged only when one is found.
Practice phone
The phone filed with the practice NPI.
Work emails
The clinician’s address at the practice, with its status.
Mobile numbers
A direct mobile number, checked as valid.
The practice line, ready to call
The phone filed with the practice NPI sits on every record that has one.
The owner’s own contacts
Work email and mobile come from healthcare waterfall enrichment, each with a status.
Credits only for results
Searching and opening practices is free; a contact uses credits only when it is found.
Data quality
How Dmand AI keeps independent practice data reliable: one NPI per practice, a clinician matched by NPI and a strict solo rule
Offices rename, take on a partner or join a group. Keying each office to its own NPI and counting who bills there keeps a solo list solo.
Illustrations: the labels Dmand AI shows on independent practice records, each on an invented record.
One practice, one NPI
Each record is keyed to the practice’s organization NPI, so a renamed office stays one account.
The clinician matched by NPI
The clinician is tied to the practice by individual NPI, never by a name match alone.
Solo means one clinician
Practices where two or more clinicians bill together are filed as medical groups, not here.
Monthly refresh and fixes on request
Practice records refresh monthly; flag a wrong phone or clinician on the profile and the data team checks it.

Questions
Independent practice and solo practice data FAQs
What buyers ask before they choose a source for data on small, clinician-run offices.
How many independent medical practices are there in the US?
Dmand AI tracks 235K+ US solo practices, where one clinician practices at one location. They include 48.2K+ behavioral health, 38.4K+ dental, 35.9K+ primary care, 24.3K+ chiropractic, 17.3K+ surgical, 14.9K+ NP or PA-led and 12.9K+ eye care practices. Practices with two or more clinicians are counted separately as 181K+ medical groups. Counts as of October 25, 2026.
What is an independent practice?
An independent practice is a medical, dental or therapy office owned by the clinicians who work in it, not by a health system, hospital or corporate chain. This page covers the smallest ones, solo practices with one clinician. Clinician-owned groups of two or more, 137K+ of them, are on the medical groups page.
What is the difference between a solo practice and a group practice?
A solo practice has one clinician; a group practice has two or more clinicians who bill under one organization. Dmand AI keeps them apart, so a list of 235K+ solo practices never mixes with the 181K+ medical groups, and a clinician who works in both shows up in both, linked.
What is a solo practitioner in healthcare?
A solo practitioner is a licensed clinician, such as a physician, dentist, nurse practitioner, therapist or chiropractor, who runs a practice alone. In Dmand AI, 230K+ solo practices carry their clinician on the record, matched by NPI.
How many independent dental practices are there in the US?
Dmand AI tracks 38.4K+ solo dental practices, where one dentist runs the office. Dental groups with two or more dentists, including those run by DSOs, are counted on the medical groups page. Counts as of October 25, 2026.
Can I find practices owned by nurse practitioners?
Yes. 14.9K+ solo practices are NP or PA-led clinics. Filter by practice type or by the clinician’s credentials, add a state, and the list shows each clinic with the nurse practitioner or PA who runs it.
What is a Type 2 NPI, and does each practice have one?
A Type 2 NPI identifies an organization, such as a practice; a Type 1 NPI identifies an individual clinician. In Dmand AI, 230K+ solo practices show their practice NPI and their clinician’s individual NPI side by side. Our guide to Type 1 and Type 2 NPIs explains the difference.
Can I see which EHR an independent practice uses?
Yes, in the app. The EHR on file is a filter, so you can list the solo practices that run the system you sell into or replace, by practice type and state. The EHR and technology data page covers the filters.
How do I contact the owner of a private practice?
Each practice record carries the phone filed with its NPI, and Dmand AI can find a work email or mobile number for the clinician who runs it. Contacts use credits only when one is found; searching and opening records is free.
Are independent practices part of a health system or medical group?
No. Solo practices sit outside Dmand AI’s org hierarchy because no parent owns them. 81.7K+ solo practitioners also practice in a medical group, and the record links to that group.
How many solo practices bill Medicare?
100K+ solo practices are enrolled in Medicare and carry a Medicare PAC ID, and their records show Part B payments where the practice bills Medicare. 136K+ are not enrolled, which is common in dental, behavioral and chiropractic care.
Where does Dmand AI’s independent practice data come from, and how current is it?
Practice names, NPIs, addresses and phones come from the CMS provider registry, Medicare enrollment and payments from CMS enrollment records and Medicare claims data, and practice type, clinician matches and EHRs from Dmand AI specialty data, affiliation data and EHR install data. Records refresh every month.
