GPO contracts: how group purchasing works for companies selling to hospitals
What a GPO contract is, how admin fees and the 3% safe harbor work, what sole-source and committed awards mean, and what being on contract does and does not do for a sales rep.
Short answer
A GPO contract is a price and terms agreement that a group purchasing organization negotiates with a supplier for all its member hospitals. Members buy at that price, and the supplier pays the GPO an administrative fee, usually up to 3%. Being on contract gives access to members, not orders: each hospital still decides what it buys.
Most writing about GPOs is for hospitals choosing one or procurement teams outside healthcare. This guide is for the company selling to hospitals: how the contract works, what it costs, what each award type does to your sales, and how to find out which GPO a hospital uses.

What a GPO contract is in healthcare
A group purchasing organization, or GPO, negotiates contracts with suppliers on behalf of hospitals, health systems and other providers. A GPO contract sets the price, the tiers and the terms that every member can use.
Nearly every hospital uses one. An industry estimate cited by the Government Accountability Office (GAO) put GPO use at 96% to 98% of hospitals, with each hospital using two to four GPOs.1
How GPOs work: member councils, RFPs and awards
A GPO does not decide alone. Members sit on councils that pick the categories to bid, set the requirements and score the proposals.
GAO describes GPO contracting in three phases: a request for proposal, review of the proposals, then negotiation and award.1 Vizient's process has more steps. Its contracting councils set requirements and decision criteria, score bids and authorize awards by consensus, and it runs 17 contracting councils and 12 advisory councils.2
The largest healthcare GPOs: Vizient, Premier and HealthTrust
Three national GPOs cover most US hospitals. Each publishes its own figures.
| GPO | Scale | How it contracts |
|---|---|---|
| Vizient | $390B a year in supply and pharmacy spend under management; 69% of US acute care hospitals work with it | Member contracting councils; bids posted publicly |
| Premier | 4,250+ acute care providers; $87B+ member purchasing volume in 2024; 3,400+ contracts with 1,400+ suppliers | Multi-source, pre-commitment and single-source contracts |
| HealthTrust | About 1,900 acute care and 90,000+ non-acute facilities in the US and UK | A committed model: members commit share for one fixed price |
Premier's figures come from its fiscal 2025 annual report, its last public filing before it was taken private by Patient Square Capital.34 Vizient's and HealthTrust's come from their own sites.56
How GPO admin fees work, and the 3% safe harbor rule
Suppliers, not hospitals, pay most of a GPO's costs. The supplier pays an administrative fee on the net price of what members buy, and the GPO shares part of it with its members.
Paying a fee to a party that steers federal health program business can be a kickback. The federal Anti-Kickback Statute makes knowingly paying or receiving one a felony.7 The GPO safe harbor in 42 CFR 1001.952(j) protects the fee when the GPO meets two conditions:8
- A written agreement with each member that states vendor fees will be 3% or less of the purchase price, or states the amount or maximum each vendor will pay.
- Disclosure to provider members at least once a year of the amount the GPO received from each vendor on that member's purchases, and to the HHS Secretary on request.
In practice, fees sit below the cap. GAO found that 3% was the most common fee at the five largest GPOs in 2012, volume-weighted averages ran roughly 1% to 2%, and the GPOs passed about 70% of fees back to members or owners.1
Sole-source, dual-source and committed GPO contracts compared
The award type decides how much selling is left after you win. GAO found that most contracts at the large GPOs were dual- or multi-source, and one GPO said about 18% of its customers' spend ran through sole-source contracts.1
| Award type | What it means | What it means for your sales team |
|---|---|---|
| Sole-source (single-source) | One supplier gets the category, usually for a deeper discount | Competitors are locked out for the term; you still win each facility's use |
| Dual-source or multi-source | Two or more suppliers share the category, with volume tiers | Every facility is a fresh competition between the awarded suppliers |
| Committed or pre-commitment | Members commit volume or a share of the category before the award | Share moves by compliance targets; GAO cites tiers as high as 80% of a category |
HealthTrust calls itself the only national committed-model GPO, where suppliers give their best pricing in return for committed market share.6 Premier runs committed programs alongside its traditional contracts.3
How long GPO contracts last, and when the next bid opens
GAO found that most contracts at the five largest GPOs ran three years, and some items, such as IV systems and analyzers, ran five to seven years.1 Miss a bid and you may wait a full term.
GPOs publish when categories come up. Premier posts a bid calendar, HealthTrust posts a quarterly bid schedule, and Vizient posts its bids publicly so any supplier can register. Track the categories you sell into and plan a year ahead.
Innovative products have a side door. All six of the largest GPOs told GAO in 2010 that their contracts let innovative products be added mid-term, and both Premier and HealthTrust run programs for new technology between bid cycles.9
How a supplier responds to a GPO RFP
The response is a formal bid, scored by the members on the council. HealthTrust's supplier path shows the usual steps.6

Answer the RFP: pricing by tier, product specifications, clinical evidence, service terms.
Negotiate and award
Councils score the bids, negotiate and award. GAO notes the large GPOs give suppliers a grievance process after awards.
What being "on contract" means for a hospital sales rep
A GPO award is a license to sell, not a sale. It gives your product an agreed price and makes it eligible for members to buy. It does not put your product on a shelf.
After the award, you still win each account:
- Tier activation. The hospital signs up for the pricing tier that matches its volume.
- Value analysis. The hospital's committee decides whether its clinicians may use the product. See how a value analysis committee works.
- A clinical champion. A department leader has to want the change.
- The distributor. If the hospital buys through a distributor, the distributor must load your contract price. When a distributor sells at a GPO price below its own cost, the manufacturer pays it the difference as a chargeback.10
GPO vs IDN: who negotiates price and who decides to buy
A GPO negotiates for many unrelated members. An IDN is a health system that owns its hospitals and sites of care.
Large IDNs belong to GPOs but often go further. They negotiate local agreements on top of GPO pricing, run their own value analysis and set standards for every hospital they own. For an IDN account, ask two questions early: which GPO does the system use, and does it contract locally for your category?
How to find which GPO a hospital uses
GPOs do not publish full member lists, so you piece it together.
- Start with the parent. Most hospitals inside a health system use the system's GPO. See how to find out who owns a hospital.
- Check announcements. Health systems and GPOs announce new memberships and renewals in press releases.
- Ask supply chain. The VP of supply chain or the contracting director will tell you, and ask which tier they buy on.
- Find the councils. Members of a GPO's contracting councils often hold supply chain or clinical roles at member systems.

Do GPO contracts cover software, IT and staffing?
Yes. GPOs contract for far more than medical supplies. HealthTrust lists IT, purchased services, human resources and capital equipment among its categories.11 In 2015, Premier awarded AMN Healthcare a three-year sole-source contract covering travel and per diem nursing, locum tenens and managed services.12
For Health IT and staffing sellers, that means two things. Check whether the system buys your category through its GPO, and check whether a competitor already holds the award. Staffing sellers should also read how MSPs and VMS platforms work in healthcare staffing.
When hospitals buy off contract
Hospitals are not required to buy through their GPO. HSCA, the GPO trade group, says hospitals and suppliers can buy and sell off contract.13
Off-contract deals are most common when a product is new and no contract exists, when an IDN negotiates directly, when a clinical team makes a strong case through value analysis, or when the hospital is not in a committed program for that category. Expect supply chain to ask why the GPO price does not work, and have the answer ready.
